Welcome to the WiTransition App!

This app will help you think about life after high school, like jobs, college or training, and living on your own and being part of your community. It connects to your Postsecondary Transition Plan (PTP), which is the part of your Individualized Education Program (IEP) that focuses on your future and is created by the time you turn 14 years old.

You have been asked to complete this app because you have an IEP. That just means you have a plan made just for you to help you in school and with your future goals.

The app helps you share your goals and dreams and lets your IEP Team and family know what support you need to reach them.

If you’re not sure about your goals yet, that’s okay! The app will ask questions to help you think about them. Some questions ask you to choose more than one answer, and others just one. Pick the answers that fit you best right now.  After you answer all questions, a report will be created that you can share with an educator/teacher, family, or anyone else that supports you in planning for your future.

When you are ready, click “Get Started.”

Here’s how to use the app!

1. Answer the questions by clicking your answers. If you need to stop in the middle that’s ok! Just come back, your progress will be saved in your browser.

2. Click “Next” to move forward or “Back” to go back and make changes. If you need to stop before you finish, your browser will remember where you stopped. Please do not clear your browser history if you leave and come back.

3. When finished, email the transition plan draft to yourself and anyone supporting you.

4. Bring a printed copy to your Individualized Education Program (IEP) meeting.

Now it’s time to share your ideas with your family and educator/teacher before your Individualized Education Plan (IEP) Team meeting. This meeting is where your transition plan will be created or updated.

Be sure to go to your meeting. It’s your chance to talk about your goals and what you need to reach them. You can even help lead the meeting if you want to!
